The supervisory authority may require the licence holder to submit a safety report.
The safety report shall include descriptions of:
1. the safety systems and equipment;
2. the measures adopted to ensure safety;
3. the enterprise organisation responsible for safety and radiological protection;
4. failures, their effects on the enterprise and the surrounding area, and their approximate frequency;
5. emergency response planning for protection of the public, in the case of enterprises as specified in Article 136.
The supervisory authority may request additional documentation.
