Licence holders must report failures in a timely manner, as follows:
- every failure: to the supervisory authority;
- failures as specified in Article 122 letter b: in addition, to the National Emergency Operations Centre (NEOC);
- failures within the area supervised by Suva: in addition, to the FOPH;
- failures leading to an exceedance of the dose limit for occupationally exposed persons in the enterprise: to Suva.
