record all of the processes which are important for safety;
maintain the records in such a manner as to be able to trace the data back to the person who donated or received the blood;
For each extraction of blood, the following shall in particular be recorded:
the surname, first name and the date of birth of the blood donor;
the date on which the blood was taken;
the test results and their interpretation.
For a person excluded from donating blood, the following shall be recorded:
the surname, first name and the date of birth;
the date and the reasons for exclusion.
For a person to whom blood or blood products are to be administered, the following shall be recorded:
the surname, first name and the date of birth;
the date of administration;
the labelling and the origin of the blood or blood products.
The Federal Council shall regulate the details. In particular, it may grant exemptions from the obligation to record in the case of autologous blood donations.
